That gap between how you feel and how your under eyes look is exactly what under eye filler is built to close.
What causes that hollow, tired look?
As we age, the fat pad and bone under our eyes lose volume. This creates a groove between the lower eyelid and the cheek. Light gets trapped in that groove and shows up as a dark shadow, even if you have no pigmentation at all.
This is called the tear trough and it’s what most people mean when they say “under eye hollows” or “dark circles that concealer won’t cover.”
Who is this actually for?
Dr Devayani explains it simply: dermal filler works best for someone who has under eye hollowing but doesn’t have a lot of loose skin, and doesn’t have very prominent under eye bags.
That said
- Mild under eye bags can also be softened with filler
- You don’t need severe hollowing to benefit even early, subtle shadowing responds well
- If you have significant loose skin or heavy bags, filler alone may not be the right fit, and that will be discussed honestly at consultation
What actually happens during treatment?
The under eye area is thin, delicate, and sits close to important blood vessels. It is treated very differently from areas like cheeks or lips.
A few things Dr Devayani insists on for this reason:
- Small amounts are used. The under eye rarely needs much product this isn’t an area where “more is better.”
- The cheek may be treated too. Sometimes the cheek just below the eye is filled first, to give the under eye area proper support from beneath rather than overfilling the hollow itself.
- Hyaluronidase is always kept on hand. This is the enzyme that dissolves filler instantly if needed. It’s a standard safety precaution during every under eye treatment, not an emergency-only measure.
Why is safety spoken openly here?
The single biggest risk with any under eye filler is accidental injection into a blood vessel, which can affect vision. This is rare, but it is the reason experience and precise technique matter more in this area than almost anywhere else on the face. It’s also why a filler-dissolving injection is always kept in the room during the procedure not as a backup plan, but as standard protocol.
Recovery what to actually expect
- Most patients resume normal work the next day
- Some healing (mild swelling or a look of tiredness) can take 3 to 4 days depending on your skin and tissue type
- Bruising, if it happens, is manageable with makeup after the first day in most cases
Being honest about results
This is the part most clinics skip, and Dr Devayani doesn’t.
What filler can realistically do:
- Visibly reduce hollowing, immediately
- Soften the dark shadow under the eyes
- Reduce the prominence of mild to moderate under eye bags
What it cannot do:
- If your dark circles are caused by pigmentation (skin colour) rather than a shadow (volume loss), filler will not fully correct them. Pigmentation needs a different approach entirely.
Knowing which one you have shadow or pigment, or often both is part of what a proper consultation is for.
An added benefit patients don’t always expect
Hyaluronic acid filler doesn’t just fill space. Placed correctly under the skin, it also draws in and holds moisture. Many patients notice their under eye skin looks more hydrated, supple, and simply healthier not just less hollow.

How long does it last?
Typically 9–12 months, though this varies by individual metabolism, skin type, and how much movement the area gets all play a role.
Can it be reversed if I don’t like it?
Yes. This is one of the few areas of the face where the product can be fully dissolved with hyaluronidase if the result isn’t right for you.
Will people be able to tell I’ve had it done?
The goal is never a “done” look it’s for people to simply stop asking if you’re tired.
